What is Support Coordination?
Let Interchange Australia help strengthen your abilities by providing assistance to understand your plan, implement supports, link you with right providers and participate more fully in the community. Our Support Coordinators are dedicated, trained professionals who will help you:
- Understand your plan
- Make informed choices
- Reach your goals
- Assist during a change in circumstances
- Connect you with supports
Support Coordination is available Australia Wide!
How to access Support Coordination
- Arrange to meet with your NDIA Planner to discuss what level of coordination of support you require to be included in your plan. In your NDIS plan, support coordination is funded by the capacity building supports budget, under ‘Support Coordination’.
- Request Interchange as your Support Coordinator. This can be done at your planning meeting or by completing and returning an Interchange referral form.
- Our NDIS Support Coordinator will contact you or your representative within three working days of receipt confirmation the outcome of your referral.
