What is a Home care Package?
A Home care Package (HCP) is a government funded package of care and services to help Australians over 65 to live independently in their own home. It is designed for those with higher care needs than the Commonwealth Home Support Program, and requires an assessment from an ACAT assessor to be eligible. Interchange Australia is an approved provider of Aged Care Services and the Support at Home program.
Am I eligible for HCP services?
You may be eligible for a home care package if you are:
- Over 65 years of age and have complex care needs
- An Aboriginal or Torres Strait Islander over 55
- An Australian permanent resident or citizen
What type of Support is available?
There are four levels of support depending on your care needs, and as the levels increase so does the funding allocated. Sometimes you will be allocated a lower level or CHSP as an interim support until a higher level package becomes available.
- Home Care Level 1 – basic care needs
- Home Care Level 2 – low level care needs
- Home Care Level 3 – intermediate care needs
Home Care Level 4 – high care needs
What Services are provided with a Home Care Package?
Home Care Packages provide older people who are able to stay at home with access to a combination of supports that assist with day-to-day activities in order to maintain their independence at home. These include:
- Personal Care
- Domestic Assistance
- Social Support
- Transport
- Gardening
- Occupational Therapy
- Physiotherapy
- Assistive Technology
- Case Management
- Package management
- Food Preparation
- Ready Made Meal Delivery
- Allied Health
- Purchase of Incontinence Aids
- Home Maintenance and Modifications
How do I Access Services?
The My Aged Care contact centre will ask you a series of questions to determine if you need an assessment by an Aged Care Assessment Team {ACAT}. During the assessment process, each individual’s priority for care is also determined. Individuals approved for a Home Care Package are placed on a national queue until a package becomes available and is assigned to them. Other types of funding can be accessed until your package becomes available. - Contact My Aged Care on 1800 200 422 to register.
- A Regional Assessment Service (RAS) assessor will contact you to organise a face to face assessment of your needs and day-to-day activities in your home.
- The assessor can then issue you referral codes for the services that suit your needs.
- You can elect to have your referral codes broadcast.
Subsidy Amounts/Supplements and Consumer Distributions
The Australian Government provides a subsidy towards the total amount of funds available under a Home Care Package. Each level of Home Care Package provides a different subsidy amount. Supplements are also available to eligible approved providers of home care to meet individual needs.
When your package is assigned, if you are not on DVA or the pension, you will receive a letter asking you to get an income and asset check. If you are already receiving OVA or the pension, you will receive a statement outlining what fees you will be asked to contribute for your Home Care Package. A home care fee estimator calculator is
