Many people want to stay at home for as long as possible as they get older and avoid or delay moving into aged care facilities or nursing homes. Interchange Australia provides a range of both government subsidised and non-subsidised supports for a range of in-home aged care services. We can provide services under the Commonwealth Home Support Programme in the Wingecarribee Shire only, and other services can be provided further afield throughout South West Sydney, the Southern Highlands, Shoalhaven and the Southern Tablelands.
Unsure which aged care program is right for you?
To be eligible for government subsidised programs you must be over the age of 65, or if you are Aboriginal or Torres Strait Islander over the age of 50. If you are under this age, you can receive non-government funded support through our privately funded program called Avec Care.
If you are 65 years of age or older, you may be eligible for subsidised supports. Depending on your care needs, services can be accessed through the Commonwealth Home Support Programme or a Home Care Package. To access these services there are 3 steps you will need to take:
1. Call My Aged Care on 1800 200 422
2. Have a home support assessment by a My Aged Care Assessor and agree on a home support plan
3. Arrange your services with a local provider
Need short term support for after hospital?
If you are looking to return home from hospital and needing help around the home, you may be eligible for Compacks services. There is no minimum age for this program. If the hospital decides that you are eligible for the program, you can receive up to 6 weeks of support once you get home for personal care, domestic assistance, transport and social assistance. Speak to your local hospital staff for more information.
